Adult Day Center - Enrollment Process

Each participant will receive a multidisciplinary assessment to determine eligibility based on Gottlieb's enrollment criteria. This assessment includes a social history, medical history, leisure survey and site visit. It is completed by a registered nurse, social worker and recreation therapist. Participation agreements, attendance schedules and fees are explained at this time.

Enrollment may include a trial period of up to one month. Staff will have frequent conversations with the family during this period to discuss progress. A non-refundable enrollment fee also is charged at this time. Upon permanent enrollment, staff will work with the new client and his or her family to create an individualized care plan.

Financial assistance is available for those who qualify through the Illinois Department of Aging's Community Care Program and the Veteran's Administrations Adult Day Service Program.
