Preparing Patients for a Successful Return Home

Our transitional care unit (TCU) provides special care for patients who have completed their hospital stay but need additional treatment, rehabilitation and recovery time before they head home.

The 32-bed certified skilled nursing unit serves patients recovering from a wide range of conditions, including stroke, heart attack, brain injury, trauma and wounds. 

The multidisciplinary team of clinicians collaborates to design an individualized recovery plan to rebuild a patient’s functional skills and ease the transition to home. In addition to a physical medicine and rehabilitation doctor, registered nurse and patient care assistant, the clinical team may include a physical, occupational and recreational therapist, speech language pathologist and registered dietitian.

 

What to Expect at the Transitional Care Unit

At Gottlieb Memorial Hospital’s transitional care unit, a treatment plan is tailored to the patient’s needs and updated as the recovery progresses.

In the first day or two, the TCU team establishes functional goals to help the patient complete the tasks he/she would like to do independently or with the help of family.

Patients take part in rehabilitation exercises and the other steps to recovery. The transitional care unit team meets to discuss progress, therapy goals and any barriers impeding improvement of functional tasks, and to establish a tentative discharge date.  The care team addresses the type of support the patient will need after leaving the hospital and recommends follow-up care, ongoing rehabilitation services, community resources and necessary equipment.

The patient is welcome to bring family to the TUC team meeting so they may ask questions or raise concerns regarding the rehabilitation.

As it nears time to leave the TCU, the patient and family and/or caregivers attend a treatment session in which the patient demonstrates his/her progress and practices what has been learned during your therapy.
